Inventions

One of the most remarkable contributions of Dr Panicker is his expertise in handling complicated and critical cases. His inventions are an extension of his experiences and knowledge. A few of his inventions are as below. Three Sections

PANICKER’S SUPER BIPOLAR FORCEPS

This is a break through invention in Electro Surgery. Any tissue or any vessel of 10 millimeter or more in diameter can be effectively and safely coagulated and divided for Open as well as Endoscopic Surgery. Burned or charred tissue will not stick to the tip of the forceps. Different models are made available. The model shown in the picture is the most versatile and ergonomic one. Because of this special shape of the forceps, it can be used for coagulation in difficult areas like the insides of the abdominal cavity, pelvis, deep in the vagina etc. This is made of precious metal, auto cleavable and last for life time and can be used with any bipolar machine.

PANICKER’S UNIVERSAL LIGATION FORCEPS

This is a modification of right angled forceps. A 60 degree curve has been introduced behind the 90 degree curve of the right angled forceps. The ergonomics, handling efficiency and utility of this instrument has increased many times by this modification. This forceps can be used as a thread carrier to isolate and tie big vessels in very deep areas. This is very useful in emergency situations to tie bleeding vessels and for Caesarian Hysterectomy in PPH without blood loss. This instrument can be used as aneurysm needle for doing Cervical Encerclage, Abdominal Hysterectomy and Non Descent Vaginal Hysterectomy. This is useful for all the surgeons.

PANICKER’S UN BREAKABLE SAFETY KNIFE [VP Handle Knife]

When a No.22 blade is inserted into this VP handle, the blade becomes unbreakable. When the hard tissues like fibroid, cartilage etc are divided the blade can break and the broken pieces can be lost in the tissue or abdominal cavity. This has happened to many surgeons. So as a permanent solution for such mishaps this safety knife has been invented. The handle is ergonomically made for a firm grip for different uses. Straight and angled models available.

PANICKER’S RADICAL D&C CANNULA

This cannulla is used for removal of Intra Uterine Polyps. This cannulla is inserted into the uterine cavity and connected to suction and a negative pressure of 400mm HG is produced. Then the cannulla is rotated clockwise and anticlockwise for a few times. This will result in radical curettage and suction of the uterine cavity.

PANICKER’S LIGHTED VAGINAL SPECULUM

This is like a one sided vaginal speculum on which a screwing device is attached. A fiber optic light cable can be directly attached to this screwing device. This can be used as a lighted vaginal speculum in vaginal surgeries, vaginal hysterectomy and suturing episiotomy, suturing vaginal and cervical tears. This can also be used as abdominal retractor with light to see clearly deep and difficult areas.

PANICKER’S PPH SUCTION HAEMOSTATIC CANNULA

This is a 25 cm long and 12 mm diameter SS cannulla with multiple holes of 4 mm diameter at the distal 12 cm [total 48 holes]. The cannulla is inserted into the post partum uterus to prevent and treat PPH. After introducing the cannulla 700 mm of negative pressure is applied. The wall of the uterine cavity is strongly sucked into the small holes of the cannulla there by closing all the bleeding sinusoids and arterioles due to pressure effect of suction. Because the wall of the uterine cavity is strongly sucked and held by the cannulla the uterus cannot expand and relax even in atonic PPH. The negative suction pressure is maintained for 30 minutes so that the clotting occurs in these small vessels and bleeding is permanently stopped. After 30 minutes the suction is released and the cannulla is slightly rotated and taken out. This is a very simple and safe break through invention to prevent and treat PPH with absolute success and connect to a suction machine and works even in DIC. This can be used during Caesarian section also.

PANICKER’S NON SLIP BULL DOG FORCEPS

This forceps is to hold and pull hard tissues like ligaments, cartilages, fibroids etc. The teeth of the forceps are aligned like the alignment of our teeth i.e. the lower jaw is inside the upper jaw. So the bite of this forceps will not slip. Very useful to pull and remove fibroid pieces when big fibroids are morcellated

PANICKER’S RAPID CERVICAL RIPENING DEVICE

This is a soft autoclavable, reusable specially made silicon device. This is a conical tubular device with 8 cm in length, 3 cm in diameter at one end and 1 cm in diameter at the other end to which a stainless steel tube in 4 cm length with a side hole is fitted. The Foleys catheter is introduced through this device and then passed through the cervical canal to reach the uterine cavity. The bulb of the Foleys catheter is inflated with 60 ml of saline and pulled down against the hole of the device and fixed to the slit on the side. The cervix is sandwiched between the Foleys bulb and the device under pressure. This pressure will enable rapid ripening and dilatation of the cervix. This cervical ripening device can be used as an adapter for PPH Suction Cannulla and MR Cannulla. It is very useful to dilate cervix in previous caesarian cases.

PANICKER’S VASCULAR / PEDICLE CLAMP

This is to hold the vessels or vascular pedicle for coagulation or tying in open as well as non descent vaginal hysterectomy. The vascular pedicle can be held with the forceps, coagulated and divided. The coagulated tip of the vessel can be further coagulated and the forceps is released slowly.

PANICKER’S LAPAROLIFT SCREW FOR GASSLESS LAPAROSCOPY

This is specially designed screwing device with a short and broad screwing portion with a T-shaped handle which is heavy and long. One limb of the T is with a hole into which a thread can be attached. During laparoscopy the screw can be screwed into the abdominal wall 3cm above the pubic symphysis after making a nick in the skin. The screw will remain beneath the rectus sheath. The thread can be tied to the right angled bar fixed on the table. This will lift the abdominal wall. Because of the heavy handle the screw will not back rotate and slip.
